The Hamilton Sports Hall of Fame (HSHOF) Public Nomination Process
The general public may nominate an athlete, team, coach, official or builder by completing the following 2011 Nomination Form.

Hall of Fame Criteria:
Candidates must be a resident of the City of Hamilton or have been a resident (defined by current boundaries) for a significant period of time, or have represented Hamilton, Ontario, Canada.
Candidates may be an individual athlete, team, coach, builder (including media) or official.
The Candidate must have demonstrated superior athletic performance or contribution. Community service may be considered.

Note: * indicates mandatory field.Athletes/Team – Detail the nominee’s competitive career as an athlete/team, including names of teams (if applicable), years of involvement, dates and names of championships attained, medals won, records and awards achieved and meritorious achievement if known.

Builder/Coach/Official – Detail the nominee’s contribution to sport in the capacity of a builder, coach or official.

Note: Details must be 250 words or less. Contact will be made by the Selection Committee if additional information is required.
